Altitude sickness can affect anyone. Being fit helps with the physical demands of the climb, but it does not make you immune to altitude illness.

What is Kilimanjaro altitude sickness?
Altitude sickness is the body’s response to reaching an elevation where less oxygen is available with each breath.
The percentage of oxygen in the air remains approximately the same as you climb, but atmospheric pressure falls. This means fewer oxygen molecules enter the body with each breath.
Your body can adapt through a process called acclimatisation, but that process takes time. When someone gains elevation faster than their body can comfortably adjust, altitude illness can develop.

Serious altitude sickness warning signs
Altitude illness can progress into high altitude cerebral oedema or high altitude pulmonary oedema.

- Increasing difficulty breathing
HACE and HAPE are medical emergencies. They require immediate action, descent and appropriate medical or evacuation support.

Why pace matters
The Swahili expression pole pole means slowly, slowly. On Kilimanjaro, it is more than a saying.
Walking too quickly increases exertion and uses energy that will be needed later in the climb. Fit trekkers can be particularly tempted to move quickly during the lower days because the terrain initially feels manageable.
The pace should be conservative from the beginning, not only after someone begins to feel unwell. Moving slowly supports acclimatisation, protects energy and gives the guiding team more opportunity to observe how each trekker is responding.
Pace does not eliminate altitude risk. It is one part of a wider strategy that also includes route choice, monitoring, food, fluids, rest and honest communication.
Hydration and eating at altitude
Dehydration can cause or worsen headaches, fatigue and dizziness. These symptoms can overlap with altitude illness, making it more difficult to understand why someone feels unwell.
Drink regularly across the day and follow the plan agreed with your guiding and medical teams. Do not wait until the evening to replace everything you have not drunk during the walk.
Hydration does not prevent altitude sickness, and drinking excessive amounts of water is not a solution. Forced overhydration can also be harmful. The goal is consistent, sensible fluid intake rather than chasing a universal number.
Appetite often falls as elevation increases, but the body still needs energy. Regular meals and manageable snacks help support walking, warmth and recovery. Tell the team early if you are struggling to eat or drink.

How is oxygen used on Kilimanjaro?

It should not be used to mask worsening symptoms so that someone can continue towards the summit. If a trekker is deteriorating, the priority is to stop ascending, assess the situation and descend when required.
