This is the distinction every first timer should understand. You can train your body for the work of trekking, but your response to lower oxygen levels still depends on time, ascent rate, sleeping altitude and individual susceptibility.

Fitness vs acclimatisation: what is the difference?
Fitness is your capacity to perform physical work. It influences how comfortably you can walk uphill, carry a daypack, control a descent and repeat the effort over several days.
Acclimatisation is the process through which your body adjusts to reduced oxygen pressure at altitude. This response begins after you arrive at altitude and requires time, which is why the structure and pace of an itinerary matter so much.
A fitter person may find a long walking day easier, but fitness does not reliably reduce their risk of altitude illness. The CDC Yellow Book 2026 chapter, High-Altitude Travel and Altitude Illness, by Peter H. Hackett and David R. Shlim states clearly that training and physical fitness do not affect the risk of altitude illness.
The UIAA Medical Commission altitude sickness factsheet reaches the same conclusion. Being fit is valuable in the mountains, but it does not protect a person from altitude sickness.
The difference can be understood through five connected principles:
- Fitness helps you perform the work.
- Acclimatisation helps your body adjust to the environment.
- Pacing helps you avoid creating unnecessary strain.
- A sensible itinerary gives adaptation time to happen.
- Honest communication helps your guides make good decisions.
These elements work together. None of them replaces the others.
Why fitness still matters at high altitude
If fitness does not prevent altitude sickness, it can be tempting to wonder why you should train at all. The answer is that altitude adds difficulty to physical work that may already be demanding.
Better fitness reduces the relative effort required to walk at a given pace. It gives you more physical reserve for cold mornings, steep ground, repeated trekking days, poor sleep and the ordinary fatigue that builds during a mountain journey.
Useful fitness for altitude helps you:
- Walk steadily for several hours.
- Climb without repeatedly exhausting yourself.
- Descend with control when your legs are tired.
- Carry the daypack required for your trip.
- Recover well enough to move again the following day.
- Manage your balance on uneven terrain.
- Maintain a calm pace without feeling pressured to rush.
Training also builds familiarity with your body and equipment. Once you know how your feet, knees, stomach, clothing and energy levels respond during a long hike, the mountain contains fewer surprises.
What fitness cannot do at altitude
Fitness cannot make the oxygen pressure at altitude the same as it is at home. It cannot force your body to acclimatise faster, guarantee that you will avoid symptoms or compensate for an itinerary that gains sleeping altitude too quickly.
Fitness also cannot predict how you will respond to altitude. Previous experience at a similar elevation and ascent rate can provide useful information, but even that is not a guarantee.
The practical lesson is not to become less fit. It is to arrive fit without believing that fitness gives you permission to ignore your body.
How to train for high altitude hiking
Most first timers should train for the real movement and duration of their chosen trip. For trekking objectives, that means steady aerobic work, strength, hills, long walks and repeated walking days rather than bodybuilding or repeated maximal effort.
Someone who already exercises regularly may be able to prepare in eight to twelve weeks. A person starting from a lower activity base should normally allow twelve to sixteen weeks, while demanding objectives such as Aconcagua or Mont Blanc may require a longer and more specific programme.
Build your aerobic base
Aerobic fitness helps you sustain a moderate effort without becoming exhausted. Brisk walking, hiking, incline treadmill walking, cycling, swimming and rowing can all help build this base.
Most sessions should feel controlled enough that you can speak in complete sentences. Hard intervals can have a place in a wider programme, but high altitude trekking is mainly a test of sustainable effort rather than speed.
A practical week could include the following elements:
- Two steady aerobic sessions lasting forty five to sixty minutes.
- One hill, stair or incline treadmill session.
- One progressively longer walk or hike.
- At least one full recovery day.
Consistency matters more than any single heroic session. Training should leave you becoming stronger over time, not permanently tired.
Strengthen your legs and core
Cardiovascular fitness alone does not prepare your joints and muscles for steep trails. Strength work makes movement more efficient and helps your body remain controlled as fatigue builds.
Useful exercises include:
- Squats.
- Reverse lunges.
- Step ups.
- Split squats.
- Calf raises.
- Glute bridges.
- Controlled step downs.
- Hip hinge movements.
- Planks and other core stability exercises.
Two controlled strength sessions each week are enough for many first time trekkers. The purpose is not to create maximum soreness or lift the heaviest possible weight, but to build durable movement that supports hours of walking.
Spend time walking on hills
Hiking is the most specific form of training for high altitude hiking. It develops endurance, balance, foot comfort, pacing and confidence at the same time.
If you do not have access to mountains, use local hills, stairs, an incline treadmill or a stair climbing machine. A flatter environment may require more creativity, but it does not prevent you from becoming ready for a high altitude trek.
Gradually extend your longest session until it begins to resemble the duration of an ordinary day on your trip. For many trekking objectives, being comfortable on a five or six hour hilly walk is a useful readiness marker.
Prepare for repeated walking days
Kilimanjaro, Everest Base Camp and Aconcagua are not single day challenges. Your body needs to recover and perform again across multiple days.
Occasionally place a shorter, easy walk on the day after your longer hike. This teaches you how your legs respond to repeated effort without requiring you to exhaust yourself every weekend.
Repeated day training should be introduced gradually. If pain changes your movement or continues to worsen, stop and seek appropriate professional advice rather than trying to push through it.
Train with the pack and equipment you will use
Carry a pack that reflects what you will actually carry on the trip. On a supported trek, this is normally a daypack containing water, food, layers, sun protection and personal essentials rather than your full expedition luggage.
Use longer sessions to test your boots, socks, trekking poles, waterproofs and food. Training is partly about fitness, but it is also the safest place to discover that a pack rubs, a boot creates pressure or a snack does not agree with you.
A practical twelve week training framework
A training plan should reflect your starting point, health and chosen objective. The following twelve week framework is a useful starting structure rather than a medical or individual prescription.
If you already have a consistent activity base and less time before departure, our six week trail ready fitness plan provides a more condensed structure. It should not be used to compress responsible preparation into six weeks if you are currently inactive or preparing for a particularly demanding objective.
Weeks one to four
Build the habit with three aerobic sessions and two short strength sessions each week. Keep most efforts comfortable and progress your longest walk towards two or three hours.
Focus on good movement, recovery and consistency. There is no benefit in beginning so aggressively that you become injured or lose motivation.
Weeks five to eight
Add more hills, stairs or incline walking while increasing your long hike gradually. Begin carrying your daypack and practise using the equipment you expect to take.
Introduce an occasional easy walk the day after your longer session. Continue strength training, with particular attention to step ups, calves, glutes, core stability and controlled descending.
Weeks nine and ten
These are your most specific training weeks. Complete your longest planned hikes, practise repeated walking days and make your training resemble the terrain and duration of the trip as closely as your environment allows.
You should finish these weeks with greater confidence, not complete exhaustion. If every session becomes a test, the programme is too aggressive.
Weeks eleven and twelve
Reduce your total training volume while maintaining light movement. Avoid unfamiliar exercises, excessive pack weight and last minute fitness tests.
Your aim is to arrive healthy, rested and ready to adapt. Fitness gained over several months will not disappear because you reduce training before departure.
Can you train for altitude at sea level?
Yes, you can build nearly all the physical fitness needed for high altitude hiking while living at sea level. You can train your aerobic endurance, leg strength, walking durability, balance, pack comfort and recovery without travelling to altitude.
What you cannot fully reproduce is acclimatisation. Genuine exposure to altitude shortly before a trip may provide some benefit, but it is optional for most first time trekking objectives and does not replace a gradual itinerary.
Altitude training masks are not the same as being at altitude. They restrict airflow but do not reduce the oxygen pressure in the air, so they do not create the same physiological environment.
The study by Porcari and colleagues, published in 2016 in the Journal of Sports Science and Medicine, found that elevation training masks function more like respiratory training devices than true altitude simulation. They may make breathing feel harder, but they do not reproduce the acclimatisation response created by genuine or properly simulated altitude exposure.
Specialist hypoxic rooms and tents are different because they alter the oxygen available. They may have uses in carefully planned programmes, but most recreational trekkers will gain more from consistent hiking, strength work, recovery and choosing the right itinerary.
Train your pace as well as your body
A common mistake is to train hard and then assume the same intensity should be carried onto the mountain. At altitude, the ability to slow down is a skill.
Use some training walks to practise moving at a conversational pace. Take shorter steps on steep ground, breathe steadily and resist the urge to race every incline.

A fit person who repeatedly pushes beyond a sustainable effort may create more fatigue and make symptoms harder to interpret. The objective is to preserve energy for the whole journey, not to win the first hill.
Hydration supports performance but does not prevent altitude sickness
Hydration matters because dehydration can cause headache, fatigue and poor performance. Those effects can make a high altitude day feel harder and may be confused with the early effects of altitude.
However, drinking more water does not force the body to acclimatise and cannot compensate for gaining altitude too quickly. Excessive drinking can also create medical problems, including dangerously low sodium levels.
Drink regularly according to your effort, the conditions and the advice of your guide or medical team. Use training hikes to learn how much you normally need, but avoid treating a fixed volume of water as protection from altitude illness.

How to know whether your fitness is ready
No single fitness test can tell you how you will respond to altitude. Readiness should therefore be assessed through several practical markers rather than one race time or gym number.
Before a major high altitude trek, you should ideally be able to demonstrate the following:
- Complete a long hilly walk appropriate to your trip.
- Carry your expected daypack comfortably.
- Walk uphill at a controlled pace.
- Descend without losing stability or control.
- Eat and drink while moving.
- Recover well after your longer sessions.
- Complete an easy walk the following day.
- Recognise discomfort without hiding it or panicking.
These markers show physical readiness, not immunity to altitude. You still need a sensible itinerary, acclimatisation time and the confidence to report symptoms early.

This is not an objective where general gym fitness is enough. Your programme should be longer, more specific and informed by the demands of your chosen itinerary.

Mont Blanc combines altitude with technical movement, steep terrain, early starts and consequential conditions. Fitness is only one part of readiness because crampon skills, efficient movement and guide judgement also matter.

Five high altitude training myths
Myth one: Fit people do not get altitude sickness
Fitness improves physical capacity but does not reliably change susceptibility to altitude illness. Runners, cyclists and experienced hikers still need to acclimatise.
Myth two: Harder training creates faster acclimatisation
Training builds fitness before the trip, while acclimatisation happens through suitable exposure and time. An exhausting training programme cannot compensate for an itinerary that ascends too quickly.
Myth three: Drinking enough water prevents altitude sickness
Hydration supports normal function and helps avoid dehydration. It does not replace gradual ascent, responsible pacing or appropriate action when symptoms worsen.
Myth four: Moving quickly proves that you are ready
A calm and sustainable pace is more useful than arriving first and exhausting yourself. High altitude rewards patience rather than speed.
Myth five: You need mountains or expensive altitude equipment
Most physical preparation can be completed with walking, stairs, strength work and consistent aerobic training. A lack of nearby mountains may require creativity, but it does not prevent responsible preparation.
The goal is not to arrive believing that nothing can affect you. It is to arrive physically ready, informed enough to respect altitude and confident enough to speak honestly.
